Thanks so much for all your support in making the 2011 Grass Roots Football Show a huge success. However we want to make the event even better next year!

To help give us shape future events we would be most grateful if you could take approx 2-3 minutes to complete the following short questionnaire, consisting of just 10 questions.

Everyone who fills out the questionnaire will be in with a chance of winning either one of 20 pairs of tickets to next year's show, or a Grass Roots Football Show t-shirt signed by all of our stars including Robbie Savage, Dion Dublin, Chris Hughton, Graham Taylor and Charlotte Jackson
